Reserve Your Spot

Schedule your child's care in advance.

Drop-offs without a reservation will be accepted only if space is available.


All required forms are necessary to ensure your child’s comfort and safety. Please read and complete each form.

Parent Handbook

It is vital that this document is read in its entirety for the success of your child’s stay. Included are state-required licensing policies, payment, drop-off, pickup, and labeling information. Please sign and initial each required section and return to the center prior to your child’s first visit. One Parent Handbook per family.


Medical Statement

The medical form along with a copy of your child's immunizations are required by your child's first visit and must be updated each year. If this form is not completed and up to date, you will not be able to make a reservation. The medical form must be signed by a physician. Your physician can fax the form directly to us. Fax numbers can be found here. One Medical Statement for each child.


Health Enrollment

This form gathers general information about your child and is required by the state of Ohio. Please sign each required section and return to the center prior to your child’s first visit. One Health Enrollment form per child.


Membership Registration

This is your registration and needs to be filled out on or before your first visit. The required annual membership fee is collected with this form. One form per family.
