All children must be fully registered members.
Registration can occur at the time of your first visit, but all forms must be completed in their entirety before a child will be accepted into Mango's Place. The medical form is required within 30 days after your child's first visit. The medical examination form must be signed by a physician. Your physician can fax the form directly to us at (614) 790-9977.

To maintain these goals, we have established certain policies and procedures that must be agreed to prior to approval of membership.
